Thank you for being an MHS track parent!  We have opportunities for you to get involved if you so desire.

#3- Volunteer to fill one of the concession slots for another sport
#4- Come to support the team.

Clothing Order

Clothing order for the 2017 season is ready!  You can find the clothing order form here.  Please remember the following:

  • The only item from this that is required is one of the required shorts.  If your primary event is going to be sprinting, purchase the sprinting shorts.  Same goes for distance and throws.  If you are unsure, talk to a coach.  If you have one of these items left over from a previous year, you do not need to buy a new one.
  • The rest of the items are not required, but we would like to see every athlete wearing something Middleton related when we go to track meets.  If you have an item from a previous year, that is perfectly fine.

Also new for this year we will be offering hats for purchase.  Please let Cory know if you want a hat.  He needs to have money from you by March 10th for you to get a hat.  He will only order the number of hats that he has money for on March 10th.  They hat is a dry fit material, grey color, with the cardinal design in the front.  Each hat is $27.

2017 Hats

Forms

There are a number of forms that are either required or that may be needed over the course of a season or school year.  All of these forms, EXCEPT THE PHYSICAL FORM, are expected to be submitted electronically when you sign-up via FEEPAY through the high school website.  There are also instructions on how to use FEEPAY on the high school website.  In the event you are unable to access a computer, we will accept hardcopy versions.  Please do not turn in hardcopy versions if you have a computer readily accessible.  Physical forms must be turned into the Athletic Office prior to the first day of practice.  A few additional forms are listed towards the end of this page:

Emergency Contact Card – – To be filled out by the parent and given directly to the coach so that it can be retained and used in the event of an emergency.

Transportation Waiver -To be used anytime a parent needs to transport their athlete home from an away contest and to be used only in the event of extenuating circumstances.

Driving Waivers

  • Self-Transportation Form – – To be used by all athletes driving themselves to a practice facility.
  • Self-Transportation Form 18 years of age or older – for those 18 years of age or older.

Concussion Forms – Required yearly as mandated by the WIAA.  There is an informational form for the parent, one for the athlete and an agreement form to be signed by both the parent and athlete and given directly to the coach.

Additional ELIGIBILITY forms can be found on the Wisconsin Interscholastic Athletic Association website at www.wiaawi.org.  Click on Schools<Eligibility Rules and Forms.  There is pertinent information regarding eligibility and you will find the following forms there:

  • Resident & Transfer Waiver Request Form
  • EVTS – Eligibility Verification of Transfer Student Form
  • Foreign Student Eligibility Verification Form – NOTE:  All Foreign Exchange students must have a U.S. physical.
